ABSTRACT

Background: Hypertension associated with proteinuria greater than 0.3g/L in a 24-hour urine collection or 1+ by qualitative urine examination, after 20 weeks of gestation is preeclampsia. The present study was conducted to evaluate prevalence of dual endocrinopathy (hypothyroidism and gestational diabetes mellitus) in patients of preeclampsia.Methods: The observational study was conducted within a period of one year from November 2015 to October 2016 in the Department of Obstetrics and Gynaecology SMGS Hospital, GMC, Jammu. A total of 400 patients of preeclampsia were included in the study and underwent serum TSH and oral glucose tolerance test.Results: In this study 32.5% of the preeclampsia patients (130 out of 400) had hypothyroidism, 15.25% patients (61 out of 400 preeclampsia) had gestational diabetes mellitus, 9.75 % patients of preeclampsia had dual endocrinopathy  (both hypothyroidism and gestational diabetes mellitus).Conclusions: The current study concluded that dual endocrinopathy (both hypothyroidism and gestational diabetes mellitus) has substantially higher prevalence (9.75%) in the patients of preeclampsia.

ABSTRACT

Peritoneal deciduosis is a rare incidental finding on caesarean section.This is a self- limited metaplastic change of submesothelial mesenchymal cells during pregnancy. We report a case of diffuse peritoneal ectopic decidual reaction discovered during caesarean section as pale whitish nodules simulating tubercles. Histology showed omentum studded with nodular groups of decidual cells with transition to spindle cells resembling myofibroblasts.

ABSTRACT

The current retrospective cross sectional study was undertaken using suspected ADR data collection form used under Pharmacovigiliance Programme of India (PvPI). A total of 2586 ADR events were recorded in 3years out of which 392(15.15%) were because of antimicrobials. males constituted 253 patients (64.54%) and females constituted 139 patients (35.45%) with male: female ratio as 1.8:1. Adults were more commonly affected followed by geriatric and pediatric population in both the groups. I.V route followed by oral route of drug administration accounted maximum ADR in similar way in both the genders. Monotherapy was responsible for 81.81% for males and 82.01% for females. Among combinations 78.26% in males and 64% in females were irrational as per latest WHO13th essential drug list. Majority of ADR, 88.14% and 92.80% were of moderate severity among males and females respectively. Maximum ADR were latent, type-A, probable in nature as per Naranjo and WHO-UMC scale. Inj.ceftriaxone followed by tab. azithromycin, tab.ofloxacin-ornidazole were the commonest antimicrobials responsible for ADRs in both the genders. The most common system involved was dermatological followed by GI in both males and females. On statistical comparison, no significant differences were observed among both the genders in any of the parameters except causality assessment scale (P<0.5). The current study suggests the ADRs due to antimicrobials are a significant health problem. No major gender related differences were observed in ADR profile of our study cohort.

Leiomyomas are frequently encountered tumors in women and have a wide and varied spectrum of presentation. We report a case of large cervical fibroid in an unmarried girl, presenting with acute abnormal uterine bleeding. Such cases pose a dilemma for the doctor as fertility preservation is a significant conern for the patient.

ABSTRACT

We present first ever report of Bullous pemphigoid induced by Tibolone, a STEAR (Selective tissue estrogenic activity regulator) that has progestogenic, some androgenic as well as estrogenic effects prescribed as an alternative to estrogen replacement therapy for treatment of climacteric symptoms in one of the 51 year old postmenopausal women with one and half year duration since menopause with previous history of use of estrogen progesterone pills during her active sexual life. The mechanism for this ADR is not well understood. But possible explanation could be progesterone activity of the drug leading to autoimmunity as reported previously. The present patient was managed by dechallenge of drug, local, oral corticosteroids and injectable, methotrexate, along with folic acid and antibiotic coverage fearing anemia and secondary infections.
